Our Case Management Services are designed to provide personalized support tailored to each client’s unique needs. We offer referrals to essential community resources, including housing advocacy, legal assistance, substance use and behavioral health services, hospice care, food and personal care items, and financial management counseling. Our team also connects clients with medical providers to help them stay actively engaged in their care and assists with obtaining medications to support their treatment plans.

In times of urgent need, we provide emergency financial assistance and coordinate transportation to and from medical appointments to ensure continuity of care. Because every client’s situation is different, our case managers work closely alongside you to ensure that the services and support you receive align with your personal goals and overall health journey.
